By scheduling with me you are acknowledging and agreeing to the following policies.
Payment
New Client appointments are required to prepay in full at the time of booking, either over the phone or via digital invoice. For all follow up sessions payment is due in full at the end of the session and may be made in the form of Venmo, cash, or credit card (some FSA/HSA cards are accepted). Prepayments or gift certificates for sessions are good for one year from issue.
Insurance
I do not accept or bill for medical insurance but can provide you with receipts if asked, so that you may submit them to your insurance carrier, if reimbursement is available to you. I can take some forms of FSA/HSA cards so please check with me if you would like to use yours.
Tipping
While tipping is common in the massage therapy profession, I consider myself a healthcare professional. As such I set my prices to be fair and compensatory with my experience so I do not accept tips for my own benefit and no tip is expected. However, should you feel inspired to offer any additional gratuity that amount will be added to a separate fund that I draw from to help pay for clients who need, but may not be able to afford, my work. Cancellations/Rescheduling
I require 48 hours notice to cancel or reschedule an appointment. If you cancel or reschedule with less than 48 hours notice you are still responsible for payment of the full session. Illness, weather, and emergencies are given generous consideration on a case by case basis. I reserve the right to require prepayment or refuse future bookings as a result of multiple cancellations or reschedules.
No Shows
Should you forget or consciously choose to forgo your appointment with no notice, it will be considered a no-show and you are still responsible for the full fee of the scheduled session. In this case I will send you a digital invoice to be paid upon receipt. I also reserve the right to require prepayment or refuse future bookings as a result of any no-shows.
Anti-Racism
I am actively working to create an anti-racist practice and a safe environment for myself and my clients. If my behavior or language results in any form of micro-aggression or racist expression, I hope you will feel safe enough to let me know so that I may apologize, work to understand my transgression, and make amends. Should you expressly or implicitly carry out discriminatory ideas or acts in any form then I reserve the right to immediately terminate our therapeutic relationship. You will not be eligible for a credit, refund, or any other compensation from me.

By scheduling with me you are acknowledging and offering your informed consent of the information below.
Consent to Treat
It is always your choice to receive massage therapy and/or manual lymphatic drainage and by scheduling with me are providing consent to receive treatment. It is understood that due to the individual nature of each client there are no guaranteed results from massage therapy or manual lymphatic drainage.
Client Responsibility
It is your responsibility to state all conditions that you are aware of to the best of your knowledge and inform me of any changes to your health as they occur. You agree to participate fully in your treatment through communication with me regarding preferences, concerns, or questions you may have. It is understood that you have the right to stop the session at any time for any reason, however, should you choose to end the session early you are still responsible for payment in full.
Scope of Practice
It is understood that a massage therapist does not diagnose any medical, mental or emotional illness, nor prescribe medical treatment, pharmaceuticals, or perform structural or spinal adjustments.
Self-Care Limitations
It is understood and agreed upon that any self-care education you receive, such as gentle stretches or lymphatic drainage hand placements, are for your individual use only. In no way are you permitted to use these techniques on another person or advise another person on their use or represent yourself as an authority as such.
Clothing & Draping
Removal of clothing is your choice and accommodated to your level of comfort. Regardless, draping will always be modest and appropriate only exposing the area of the body being worked on/with. Certain work, such as scar tissue release post-mastectomy, may require undraping of the breast but will be discussed and only done with your express consent. you agree to bring any questions or concerns about your comfort level to me attention as needed.
Communications & Marketing
By scheduling with me, Amy Hartl, LMT, you give permission to receive occasional newsletters and emails from Amy Hartl, LMT and As We Are Now LLC. It is understood that you can unsubscribe from these emails at any time without impacting your ability to receive appointment confirmations and reminders or other information relevant to your therapeutic relationship with Amy Hartl.
